ABA therapy with NC Medicaid

NC Medicaid covers ABA therapy for children with autism, whether your child has NC Medicaid Direct, a Standard Plan, or a Tailored Plan. We check your coverage for free and handle the prior approval with your child's health plan.

Who is eligible

NC Medicaid covers ABA therapy as part of Research-Based Behavioral Health Treatment (RB-BHT) for autism spectrum disorder (ASD). Your child qualifies when they:

  • are enrolled in NC Medicaid, through NC Medicaid Direct, a Standard Plan, or a Tailored Plan,
  • have an ASD diagnosis made with a scientifically validated tool, such as the ADOS-2, CARS-2, BOSA, or TAP, and
  • need ABA for medical reasons, as shown by a written assessment.

A licensed psychologist, a licensed psychological associate, or a physician makes the diagnosis. A screening tool, a school’s educational determination, or an informal impression alone is not enough to start ABA. Children under three can start with a provisional diagnosis, as long as a full diagnosis follows within six months.

For children and young adults under 21, Medicaid’s Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) benefit also applies. EPSDT can cover medically necessary care beyond the usual limits of a policy, but it does not remove the need for prior approval.

Tailored Plans

Tailored Plans are NC Medicaid health plans for people with serious mental illness, severe substance use disorders, intellectual or developmental disabilities, or traumatic brain injuries. They are the only Medicaid plans that offer the NC Innovations Waiver. If your child has a Tailored Plan, ABA is approved through that plan, and your child’s Tailored Care Manager can help coordinate it with your child’s other services.

How to start ABA therapy with NC Medicaid

  1. Get the diagnosis. A licensed psychologist, licensed psychological associate, or physician diagnoses autism using a validated tool.
  2. Get the service order. A physician or licensed psychologist signs a service order for ABA. The order must be in place before therapy begins.
  3. Assessment. Our BCBA completes the assessments the policy requires, including a skills assessment such as the VB-MAPP or ABLLS-R and an adaptive behavior assessment, plus a functional behavior assessment when challenging behaviors are part of treatment.
  4. Prior approval. We send the treatment plan and assessments to your child’s health plan, or to NC Medicaid for NC Medicaid Direct, and request prior approval. Services delivered without it are not covered.
  5. Therapy starts. Your child’s sessions begin in your home, in our clinic, or in the community, based on the treatment plan.

Once services start

The BCBA reviews and updates the treatment plan before each authorization ends: at least every 180 days for 16 hours a week or fewer, and at least every 90 days for more than 16 hours a week. The service order is renewed every year.

Caregiver training is part of every NC Medicaid treatment plan, with at least six sessions in each 180-day period. Some services can be delivered by telehealth when they follow NC Medicaid’s telehealth policy.

The treatment plan also includes a plan for reducing hours over time as your child’s skills grow and carry over to home, school, and the community.

NC Medicaid ABA therapy FAQs

Yes. NC Medicaid covers ABA as part of Research-Based Behavioral Health Treatment (RB-BHT) for autism spectrum disorder, under Clinical Coverage Policy 8F. Your child needs an autism diagnosis, a signed service order, and prior approval before therapy starts.

The ABA benefit follows the same state policy in each. What changes is who approves it: a Standard Plan or a Tailored Plan approves ABA for its own members, and NC Medicaid Direct covers beneficiaries who are not enrolled in NC Medicaid Managed Care. Your Medicaid card names your child's plan, and our intake team can look it up with you.

Yes. WellCare of North Carolina's Medicaid plan merged into Carolina Complete Health on April 1, 2026. Members moved to the combined plan automatically and received a Carolina Complete Health ID card. Carolina Complete Health is now a statewide plan, and we accept it.

Only if your child is under three years old when services start. In that case NC Medicaid accepts a provisional diagnosis, and your child needs a full autism diagnosis within six months. Older children need a full diagnosis made with a validated tool such as the ADOS-2, CARS-2, BOSA, or TAP.

A licensed physician (MD or DO) or a licensed psychologist signs the service order. It must be in place on or before the first day of services, and it is valid for one year. Medical necessity is reconfirmed and the order renewed every year.

It depends on the hours in the treatment plan. A plan of 16 hours a week or fewer can be authorized for up to 180 days at a time. A plan of more than 16 hours a week is authorized for up to 90 days and reviewed for reauthorization at least every 90 days.

Yes. The treatment plan has to schedule at least six caregiver training sessions in each 180-day authorization period. Your BCBA uses these sessions to teach you the strategies your child is learning, so progress carries over at home.
